Hello doctor, my mother is 68 years old. In mid-June, she was diagnosed with gastric adenocarcinoma with lymph node metastasis to the liver area. The gastric tumor is about 5 cm in size near the pylorus, and a similar 5 cm tumor was found in her liver. At the end of July, the hospital placed a stent in her stomach to improve her eating. She is scheduled to undergo her first chemotherapy session on August 25th, using XELOX (intravenous injection + oral administration), which is the first-line treatment recommendation. The public hospital doctor advised using 80% of the recommended dose due to her advanced age and low weight (currently only about 36 kg). (We are considering using traditional Chinese medicine as an adjunct therapy to reduce side effects). However, a private doctor suggested using another chemotherapy drug, TS-1, which can be taken orally for palliative care in advanced gastric cancer. Public hospital doctors all say that oral TS-1 has fewer side effects than XELOX, but they are unsure about the efficacy. Both can be used… Question: I don't want my mother to suffer too much physical harm. She is malnourished and weak and cannot tolerate the treatment. We still hope she receives the correct treatment. Is it possible to use TS-1 (with traditional Chinese medicine as an adjunct)? Can TS-1 be used alone, and how common is it? What should we do?

My father (68 years old) was diagnosed with stage IV stomach cancer at the end of March: an approximately 8.5cm gastric tumor with infiltration of the spleen and the distal pancreatic body and tail, up to 4.9cm diffuse liver metastases, metastatic lymph nodes seen in the upper abdomen, and a small amount of lower pelvic ascites. Is palliative care the only option? Also, my father has had a cough without phlegm for about a week and only saw his family doctor yesterday. However, he did not mention his cancer diagnosis at that time. Today, after taking the prescribed medications (Dhatifen, Ploton, Ipradol, Codaewon Tablet, and Casirol), he experienced unsteadiness and dizziness. Is this a symptom of common cold and flu medication or a sign of cancer metastasis? Thank you!
